Insurance At Victory Spine and Sport

We accept many insurance plans and do our best to keep the process simple.
If you’re not sure what your plan covers, we can help you understand your benefits and what to expect before you start care.

Important: Coverage varies by plan. We’ll provide an estimate based on the benefits we verify, and we recommend you also confirm coverage directly with your insurance carrier for the most accurate details.

Our goal is straightforward: no confusion, no surprises, and clear next steps.

How Insurance Works Here

  • Electronic intake before you arrive: We send forms to collect your information (including insurance details) ahead of time.
  • Benefit verification: We review and verify benefits whenever possible before your first visit.
  • Clear estimate: We explain what we found (copay, deductible, visit limits, and expected costs).
  • Keep it moving: We run a tight schedule and do our best to minimize waiting.

Out-of-Network, HSA/FSA, and Self-Pay Options

Out-of-network: Some insurance plans include out-of-network benefits even if we’re not in-network.
If you’re unsure, contact the office before your first visit and we can help you check what your plan offers.

HSA/FSA: Many patients use HSA/FSA funds for care (including out-of-network care). Plan rules vary, so it’s always a good idea to confirm with your provider.

Not using insurance? We offer a time-of-service discount for paying at the time of your visit.

Insurance FAQs

Do I need a referral?
No. If your plan requires one, we’ll let you know what we find and what steps to take next. Currently, only some Kaiser plans require a referral.

Can you tell me exactly what I’ll owe?
We provide an estimate based on the benefits we verify, but the final determination always comes from your insurance carrier once a claim is processed.
If you want the most accurate confirmation, call the number on your insurance card.

What information do you need to verify benefits?
Your insurance provider, member ID, and date of birth. Our electronic intake makes this easy to submit before your first visit.

What if my insurance isn’t listed?
Reach out anyway. We’ll help you check benefits and go over options (in-network, out-of-network, or self-pay) so you can decide what makes the most sense.
